Autopsy results showed a grossly diseased and very small brain weighing 84 g, which is 4 standard deviations below average; an absence of cerebral gyri or brain folds; severe dilation of both cerebral lateral ventricles; and dystrophic calcifications throughout the cortex, the subcortical white matter, and the stem and spinal cord. Zika virus was found in the fetal brain tissue on reverse transcriptase–polymerase chain reaction assay, and the complete Zika virus genome was recovered from the fetal brain.
Fetal body weight was 1470 g (5th percentile), body length was 42 cm (10th percentile), and head circumference was 26 cm, which was in the first percentile.
Calcifications in the placenta and a low placental–fetal weight ratio indicate the virus may have damaged the placenta. The virus was not found in other organs beyond the brain.
Dr.Rubin
In an accompanying editorial, Eric J. Rubin, MD, PhD, from the Department of Immunology and Infectious Diseases, Harvard School of Public Health, Boston, Massachusetts, and colleagues point out that although the autopsy findings make the link stronger, the report does not prove that Zika virus causes microcephaly. The accepted criteria for proving causation, they note, were those set by Robert Koch in 1890, "which require the isolation of the causative organism, reinfection of a susceptible person in whom the characteristic disease develops, and then repeated isolation of the organism."
The authors have disclosed no relevant financial relationships. One editorialist reports receiving grant support from the Ragon Institute, the National Institutes of Health/National Institute of Allergy and Infectious Diseases, and the Gates Foundation outside the submitted work. He is also a deputy editor at the New England Journal of Medicine and is involved in HIV vaccine clinical trials conducted in collaboration with the National Institutes of Health, HIV Vaccine Trials Network, International AIDS Vaccine Initiative, Crucell/Janssen, Military HIV Research Program, Gates Foundation, and the Ragon Institute. The other editorialists have disclosed no relevant financial relationships.
