今日はセカンドステップシリーズ（不妊治療）の第四弾です。
I want to write another article for The Second Step series (Infertility treatment) today.

さて、最初に受けた血液検査で、TSH（甲状腺刺激ホルモン）の数値が高かったHit-chan。
So, according to the last result of my blood test, my TSH (Thyloid Stimulating Hormone) was higher than the normal range.
その後、担当看護師のパットさんが紹介してくれた、同じ系列病院の内分泌科のドクターの受けに行きました。
After that, my nurse, Pat, wrote me a letter for a Park Nicollet endocrinology clinic.
今回も不妊に関わるという事と難しい話が出るかもしれないという事で、Andyっちに付いて来てもらいました。
I asked Andy to come with to the appointment this time again since it's relating to our infertility and the medical conversation might go too difficult for me to understand.
予約をしてから、手紙で予約確認が届いたりしたんですが、道路工事が多い夏のミネソタ、どうやら病院までの大きな道も工事で閉鎖されているらしく、「○○の道は閉鎖されているので、ご注意を！」という紙が入っていました。
（丁寧なサービスにちょっとビックリ。笑）
After I made the appointment, they sent me a couple of reminder letters, but they also put a note which says "Road 100 is under construction!" in the envelopes.
(I was so impressed by the kind service.)
で、早めに家を出て、病院へ。。。
We left home early enough that morning.
大きな病院で、建物がいくつも分かれていたりしたんですが、事前に調べておいたので無事、言われた通りの予約時間15分前には受付に辿りつけました。
The Park Nicollet was so huge and had many buildings, but we reviewed the address and the route beforehand so that we could arrive 15 minutes before my appointment.
受付を済ませて待っていると、看護師さんが呼びにきてくれました。
After I signed in, a nurse called my name.
今回の看護師さんは明るくて優しい方だったので、良い感じで体重、血圧測定、その他簡単な問診を済ませる事ができました。
She is such a outgoing and kind lady and made the first process like measuring my weight or blood pressure pretty easy.
その後、ドクターDが現れました。
After that, Dr. D showed up in the room.
予約の時に、女性ドクター、男性ドクターのどちらが良いか聞いてもらったのですが、Hit-chan、全くもって気にしないタイプなので、どっちでも良いですと言ったら、男性ドクターDに当たりました。
When I made the appointment, they kindly asked me if I would prefer a male or female doctor, but I was easy and totally open to either way. Then, the male Dr. D become my doctor.
で、良いドクターに出会えるかどうかは、ある程度「運」のようなものがあると思うのですが、その点で言えばHit-chanは運が良いと思います。
I guess it is kind of depending on luck if you can meet nice doctors or not and if I see it that way, I've been totally lucky so far.
Hit-chanを担当してくださるドクターDは落ち着いた方で、部屋に入って来てすぐに、「どうも、遅くなってすみません。前の患者さんに時間がかかってしまって。」と挨拶してくださいました。
（そんなに待ってなかったのにね。）
Dr. D is really calm, down to earth and so polite. When he came in the room, he greeted us saying "Sorry to make you wait. I took time to see the patient before."
(We actually didn't wait that long.)
しかも、その後「昨日の夜と今日も、カルテを読んでいたんですが・・・」と仰り、その通り、もう既にカルテに書かれている事をもう一度説明する必要は全くありませんでした。
Besides that, he mentioned "I reviewed your information last night and this morning too..." in the beginning, so we didn't have to start explaining things which are already on my chart over again.
もう、この時点でHit-chan、「お～、ドクターD、好き。」ってなっていました。(笑)
At that point, I was already like "Oh, I really like Dr. D!"
その後、色々と問診や触診を受けた後、やはりHit-chanのTSHが上がっているのは放射線治療を受けた影響の可能性が強いとの事でした。
After he asked me some questions and checked my thyroid, he said that my thyroid problem was probably from the radiation therapy that I had in 2007.
ただし、数値自体は標準範囲を大きく逸脱するものではないので、少なめのお薬を飲むところから始めましょうとなりました。
Still, my TSH number is not outrageously off from the normal range, so he recommended me to start from taking a little amount of medicine.
あと、甲状腺と不妊の関係性についてですが、ドクターDは
Speaking of the interrelation between thyroid abnormality and infertility, Dr. D said
「色々な資料を読む限り、不妊と甲状腺機能の関係を示す確かな根拠は見つかってないんだけど、僕が診てきた患者さんでも、やっぱり甲状腺機能を平常に戻したらすぐに妊娠したカップルは何組もいるから、やっぱり何か関係していると考えられるね。」
"As far as I've read a lot of things, nothing says that thyroid problem causes infertility, but I know a lot of my patients got pregnant right away after they fixed the thyroid problems, so I assume there's probably something related between them."
と仰っていました。
あと、普通の場合なら、TSHの値は0.2-4.5の範囲にあれば正常なんですが、妊娠を考える時は、その半分の0.2-2.0くらいにするのが望ましいとの事でした。
Also, he mentioned that TSH normal range is usually 0.2-4.5, but if you want to be pregnant, you should keep the number under 2.0 or so.
ドクターD、本当に優しい上に賢くて、おそらく本当はもっと難しいであろう医学的な内容も、AndyっちとHit-chanにも分かりやすく、噛み砕いて丁寧に教えてくださいました。
（Andyっちも一緒に来て、説明を聞いてくれて本当に良かったです。）
Overall, I was just so impressed how smart Dr. D was. I assume that the real medical explanations would be more difficult, but he was excellent at explaining those difficult matter in a very easy way for us.
(I'm so glad that Andy was there and listened to the great explanation about my problem.)
最後に、この日もう一度採血を行って、それを踏まえて処方箋を書いてくださるという事になりました。
At the last, Dr. D had me have another blood test that day and said that he's going to write a prescription based on the result for me.
という訳で、不妊治療のステップ、また一つ前に進む事ができました。
Okay, this is how we made our infertility treatment 1 more step forward.

診察室に入ってきて最初の一言で、「あ、この先生仕事ができる！」って分かるくらい、素敵な先生でした。賢い人でも、凡人に上手く説明できるかどうかは別問題だと思いますが、私の先生は本当に説明も上手で尊敬しっぱなしでした。
道路工事、やっていない道路はないくらいですよね。幸い私は家から近い所に勤務なので良いですが、毎日通る道が工事してるとストレス溜まりますよね。他のドライバーもイライラしてるし、とっても悪循環。
早く工事終わる事を祈ってます！
