 Alhydrogel(水酸化アルミニウムゲル)は、プラスに荷電していて粒子サイズが小さい

Adju-Phos(リン酸アルミニウム)は、マイナスに荷電していて粒子サイズがより大きい

Alhydrogel(水酸化アルミニウムゲル)は、注射箇所では、プラスに荷電される。筋肉に含まれるリン酸塩と反応するためと思われる。

接種後に、免疫細胞に貪食されるサイズは、2.7μm以下と考えられるので、Alhydrogel(水酸化アルミニウムゲル)は約97%が貪食され、Adju-Phos(リン酸アルミニウム)は約20%が貪食されるであろう。

Causal relationship between immunological responses and adverse reactions following vaccination.

Abstract

Vaccine adverse events and controversial safety issues have occurred in recent decades in Japan: aseptic meningitis following the measles-mumps-rubella combined vaccine (MMR), anaphylaxis after immunization with live virus vaccines and inactivated split influenza vaccine, an increased incidence of febrile illness following the simultaneous administration of inactivated vaccines, and chronic pain with neurological illness after immunization with the human papilloma virus vaccine (HPV). Vaccine adverse events are a matter of concern for the public as well as general practitioners; some are within the range of assumptions that adverse reactions after live attenuated vaccines are related to the nature of their parental wild-type viruses. Vaccines stimulate the innate immunity of host immunological defense mechanisms and induce the development of specific acquired immunity. Some adverse events related to autoimmune responses have been reported, such as idiopathic thrombocytopenic purpura and acute disseminated encephalomyelitis (ADEM). Although a plausible relationship was not demonstrated, the possibility of an association cannot be denied. The pathogenicity of adverse reactions was investigated for anaphylactic reactions, systemic and local reactions following vaccinations. Initial innate immune responses are essential for the development of acquired immunity and are related to adverse events from different viewpoints.

Severe Herpes Zoster Following Varicella Vaccination in Immunocompetent Young Children.

Abstract

Varicella vaccination is now virtually universal in North America, as well as in some European and Asian countries. Since varicella vaccine is a live attenuated virus, the virus replicates in the skin after administration and can travel via sensory nerves or viremia to become latent in the dorsal root ganglia. In some immunized children, virus reactivates within a few months to a few years to cause the dermatomal exanthem known as herpes zoster (shingles). Herpes zoster caused by vaccine virus often reactivates within the same dermatome as the site of the original varicella vaccine injection. We present evidence that occasional cases of herpes zoster following varicella vaccination in immunocompetent children can be as severe as herpes zoster following wild-type varicella. Analysis of the virus in one case disclosed that the vaccine virus causing herpes zoster was a wild-type variant with a mutation in ORF0. With regard to dermatomal localization of the viral eruption, we predict that herpes zoster of the lumbar dermatomes in children is likely to be caused by vaccine virus, because herpes zoster in those dermatomes is rare in children after wild-type varicella. One of the children with herpes zoster subsequently developed asthma, a known risk factor for herpes zoster, but none of the children had an autoimmune disease. Although postherpetic neuralgia is exceedingly rare, children who develop herpes zoster following varicella vaccination are at risk (albeit low) of developing meningoencephalitis and should be carefully observed for a few weeks.

Herpes Zoster Involving the S1 Dermatome

Herpes zoster is unusual in persons younger than 10 years of age, and when it does occur in this age group, the lesions have a predilection for dermatomes supplied by the cervical and sacral nerves.

ワクチンの非特異的効果の頗る悪いDTPワクチンの被害を厚労省のサイトで調べてみたら、5ヵ月齢の男子3人がエヴァンズ症候群なるものを発症していた。

厚労省の説明

自己免疫性溶血性貧血(AIHA)は、赤血球膜上の抗原と反応する自己抗体が産生され、抗原抗体反応の結果、赤血球が傷害を受け、赤血球の寿命が著しく短縮(溶血) し、貧血を来す病態である。

(1)温式AIHA…臨床像は多様性に富む。特に急激発症では発熱、全身衰弱、心不全、呼吸困難、意識障害を伴うことがあり、ヘモグロビン尿や乏尿も受診理由となる。症状の強さには貧血の進行速度、心肺機能、基礎疾患などが関連する。代償されて貧血が目立たないこともある。黄疸もほぼ必発だが、肉眼的には比較的目立たない。特発性でのリンパ節腫大はまれである。脾腫の触知率は32~48%。特発性血小板減少性紫斑病(ITP)を合併する場合をエヴァンズ(Evans)症候群と呼ぶ。

Tx w/IVIG & improved. D/C to home on continued steroids w/central line for future IVIG tx. D/C to home on steroid taper. 4/3-4/4 Admit for IVIG. Plts 35K. 4/6 Seen in clinic for URI & left OM. Tx w/oral antibiotics 4/7-4/8 Admit for IVIG. Plts 21K. 4/10-4/17 Admit for vomiting, increased bruising, irritability, blood tinged stool & falling plt ct. Tx w/IVIG 4/22 Clinic visit for resolved OM. 4/26 Seen in ER for episode of irritability.

ワクチンの非特異的効果

ワクチンには、目的とする病気に対する効果以外にも、健康に利する効果と健康を害する効果があり、これらの効果を、非特異的効果と呼ぶ。

西アフリカで小児科医師として働く、クリスティン・スタベル・ベン医師は、過去25年間のワクチン接種歴と乳幼児の生存率や入院率を比較し、ポリオ生ワクチンの接種で、乳児の生存率が増加すること、DTPの接種が生存率を低下させることを示す論文を投稿したが、掲載してくれる学術誌を見つけるのに苦労していると話している。

生ワクチンを接種することで、新生児の免疫系が刺激され、その後の感染症に対する抵抗力がつくことは、彼女たちのグループ以外でも示されている。ポリオ生ワクチン以外でも、BCGや麻疹ワクチンでも同様の傾向がみられている。

一方、不活化ワクチン接種がその後の生存率を低下させることや入院率を増加させる結果は、他のチームからも報告されている。

不活化ワクチン接種の負の効果は、女児において顕著にみられる。

The non-specific effects of vaccines and other childhood interventions: the contribution of INDEPTH Health and Demographic Surveillance Systems

Most childhood interventions (vaccines, micronutrients) in low-income countries are justified by their assumed effect on child survival. However, usually the interventions have only been studied with respect to their disease/deficiency-specific effects and not for their overall effects on morbidity and mortality. In many situations, the population-based effects have been very different from the anticipated effects; for example, the measles-preventive high-titre measles vaccine was associated with 2-fold increased female mortality; BCG reduces neonatal mortality although children do not die of tuberculosis in the neonatal period; vitamin A may be associated with increased or reduced child mortality in different situations; effects of interventions may differ for boys and girls. The reasons for these and other contrasts between expectations and observations are likely to be that the immune system learns more than specific prevention from an intervention; such training may enhance or reduce susceptibility to unrelated infections. INDEPTH member centres have been in an ideal position to document such additional non-specific effects of interventions because they follow the total population long term. It is proposed that more INDEPTH member centres extend their routine data collection platform to better measure the use and effects of childhood interventions. In a longer perspective, INDEPTH may come to play a stronger role in defining health research issues of relevance to low-income countries.

Evidence of Increase in Mortality After the Introduction of Diphtheria–Tetanus–Pertussis Vaccine to Children Aged 6–35 Months in Guinea-Bissau: A Time for Reflection?

 

Conclusion

Although having better nutritional status and being protected against three infections, 6–35 months old DTP-vaccinated children tended to have higher mortality than DTP-unvaccinated children. All studies of the introduction of DTP have found increased overall mortality.

最後に、上記の結果を踏まえて、アラムによって誘導される細胞内DNA認識による自然免疫活性化がどのような細胞を介して誘導されているか検討しました。その結果、IgEが産生されるような反応はinflammatory monocytes (iMONOs:炎症性単球)がアラムによって誘導されたDNAによるIRF3を介した自然免疫反応を惹起してinflammatory dendritic cells (iDCs:炎症性樹状細胞)に成熟すること、そして その細胞が接種部位から所属リンパ節への移動すること が必須であることが判明しました。また、そのプロセスはI型インターフェロンではなく、IRF3で誘導される別のサイトカイン、IL-12p40であることも判明しました。IL-12p40はホモ2量体をつくり、IL-12p80となると、Th1を抑制し、Th2タイプの免疫反応を誘導することが知られており、実験的にもこのサイトカインが上記の反応に重要であることも今回の研究で示すことができました。
