健康だった 51 歳の女性がファイザーワクチン2回目接種の翌日、高熱、頭痛、異常な性器出血を経験した。接種の5日後に来院。COVID-19患者との接触歴なし。臨床検査で血小板減少症と診断(7.3 × 104 /μL)、フィブリン分解生成物上昇 (86.5 μg/mL)、D-ダイマー (39.28 μg/mL) mL)、乳酸脱水素酵素 (LDH) (812 U/L)、およびフェリチン (7644 ng/mL) レベル。血栓症、梗塞、出血または異常な肺野の不透明度なし。SARS-CoV-2 PCR、血液培養、抗核抗体、リウマチ因子、 カルジオリピン抗体はすべて陰性。アストラゼネカワクチン接種後の報告があった血栓性血小板減少症を考慮し、アルガトロバンとIVIgを開始。結果的に、抗血小板 第4因子抗体は陰性であった。しかし、全身状態と臨床検査 LDH、フェリチン、トランスアミナーゼが悪化。したがって、ステロイドパルス療法(メチルプレドニソ 1日1gを3日間単独で)を開始、その後ワクチン接種後のMASの診断後経口プレドニゾロンを服用。
退院の日にも、IL-18のレベルが依然として異常に高い???
今年の1月に、脂質ナノ粒子の中のカチオン化可能脂質が炎症反応を引き起こすことを証明している論文を当ブログで紹介しました。
もしかすると、カチオン化脂質が自然免疫系に認識される仕組みがあるのではないかと思っています。例えば、バクテリアの膜の中のある成分(lysyl-phosphatidylglycerol)の一部が、カチオン化しているのではという意見があり、このカチオン脂質をヒトの免疫が異物と認識できるようなシステムがあるのではないかということですが。
It remains to be determined how these LNPs or their ionizable lipid component activate distinct inflammatory pathways. In theory, LNPs could activate multiple pathways or alternatively engage only one that would initiate an inflammatory cascade. Some cationic/ionizable lipids bind and activate TLRs (Lonez et al., 2012, 2014; Samaridou et al., 2020; Tanaka et al., 2008; Verbeke et al., 2019). Our GSEA analyses revealed that these proprietary LNPs are also likely to
activate TLR pathways, among others (Figure 2F). We also observed egulation of inflammasome components such Nlrp3 and enrichment of genes involved in necroptosis. In concordance with these data, our flow cytometry data revealed significant cell death after inoculation with LNPs. Inflammatory cell death, such as necroptosis and pyroptosis, could cause the release of DAMPs and the further enhancement of inflammation.
