グラム染色道場の師範様からのペーパーをざっと読んでみました。

Bacteriologic analysis of infected dog and cat bites [NEJM 1999;340:85-92]

です。110人の患者をenrollした多施設のprospective studyです。
polymicrobial infectionが6割近くを占めPasteurellaが主要なpathogenとして
大事だと思いました。犬では50%, 猫では75%を占める原因菌となりうるようです。
犬ではPastuerella canis が、猫では Past.multocida subspecies であるmultocida とsepticalがpathogenとしてこのstudyでは多かったです。
次に重要なのはStreptococci, staphylococci, moraxella, corynebacterium, and
neisseria あたりのようです。
*** 一方でブ菌やStrep. pyogenesは犬、猫の感染の原因には比較的なり難いという記載もありました。Staph.aureus was present in 20 percent of dog bites and 4 percent of cat bites, and Strep.pyogenes was present in 12 percent and 0 percent, resptctively.
*** Anaerobeはfusobacterium, bacteroides,porphyromonas, prevotellaあたりが原因とのこと。トラに噛まれた症例が除外されていたのは笑いでした。
*** Invasive zoonotic pathogen : Capnocytophaga species, W. zoohelcum
が挙げられてました。rareですが、、治療に関してはantistaphylococcal penicillins, first-generation cephalosporins, clindamycin and erythromycinなどはPasteurellaに対しては効果が弱い可能性があるので第一選択には薦められないようです。AMPC/CVAが第一選択でしょうか、、上記に関してEditorialでは面白いことが書いてありました(以下参照)

抗菌薬がinfection自体を予防するかどうかはcontroversialであるが、infection
のリスクを下げるというstudyが幾つか出ている。high-risk wounds, such as deep punctures(particularly if inflicted by cats), those that require surgical repair, and those involving the hands.では予防投与を推奨している。では、、、、high-risk woundsとは、、、、BMJの先の論文によると
■***Indications for antimicrobial prophylaxis***
●High risk wounds
*All bite wounds after primary closure
*Puncture wounds
*Bites to hand and wrist
*Crush wounds with devitalized tissue
*Dog bite injuries to the genitals
●High risk patients conditions
*Diabetes mellitus
*Immunosuppression
*Splenectomy, cirrhosis(C canimorsus)
*Postmastectomy
*Rheumatoid arthritis and prosthetic joints
とのこと。
ところで例のCapnocytophagaについてもNEJMには以下の記載が、、

When fever occurs in immunosuppressed patients after a dog bite, the possibility of an infection with Capnocytophaga canimorsus, an invasive organism, should be considered. Capnocytophaga species were recovered from 4.7 percent of the qounds cultured by Talan et al.
その他、頭に浮かべておいた方がよさそうな疾患も述べられている。

写真は当直中に昔、はも、に噛まれてやってきた職人さんがいましたが、思わず縫っただけで何もしなかったのを思い出しました。歯が鋭くて痛そうです。最近では釣り針が刺さった人が当直で来ましたが、あれは、返し、があるので抜けないので、麻酔して針を押し出して抜きました。
