「（PETで）増加する部位は理解できるが、脳全体の血流は変化が無いので、増加した部位とは別の部位の血液が減少することが知られていた。それらの部位は、賦活試験の種類に関わらず、同じような部位の血流が減少することが明らかになり、ワシントン大学のDr.Raichleがactivationの反対という意味でdeactivationとして報告した。それらの部位を詳細に検討すると、アルツハイマー病で障害を受けやすい部位に相同であることが明らかになってきた。
　fMRIのデータでも同様の結果が得られるため、fMRIでも研究がおこなわれてきたが、fMRIの場合、deactivationというよりも酸化ヘモグロビンと還元ヘモグロビンの差を見ているので観察しているものが異なる。また、さまざまな異なった賦活試験で同じ結果が得られたため、deactivationよりもdefault modeと呼ばれるようになり、いくつかの部位がネットワークのように同じパターンで出現するので、default mode networkと呼ばれるようになった。」

鍼鎮痛の理論

　エツァート・エルンストの『鍼治療の科学的根拠』（医道の日本社、原著は1999）や、アドリアン・ホワイトの‘An Introduction to Western Medical Acupuncture’やあるいはDavid Bowsherの‘Mechanisms of acupuncture’で鍼鎮痛の理論を研究するとクリアーに理解しやすい。

　まず、脊髄レベルでの「ゲート・コントロール」での鎮痛。
それに、脊髄から上行し、
脳幹（延髄・橋・中脳）の
網様体（reticular formation）、
大脳辺縁系（limbic system＝扁桃、海馬、帯状回）、
視床下部の弓状核（hypothalamic arcuate nucleus ）、
視床（thalamus）から、
前頭前皮質（Prefrontal cortex）、
大脳皮質感覚野（Sensory cortex）に行く。

 視床下部の弓状核（hypothalamic arcuate nucleus）が上行系と下行系の交わるところである。
　視床下部弓状核のドーパミン・ニューロンが視床下部の腹内側核（ventromedial nucleusまたはventromedial hypothalamus：VMH）に連絡し、ここから縫線核（raphe nuclei）系のセロトニン系と傍巨大神経細胞網様核（paragigantocellular reticular nucleus）を経由するノルアドレナリン系の下行性痛覚抑制系によって鎮痛が発現する。

　中脳水道周囲灰白質（PAG;：periaqueductal gray matter）の電気刺激によるSPA（stimulation produced analgegia：刺激による鎮痛）は、鍼鎮痛と明らかに関係があるとされている。武重千冬教授によれば、ツボを電気刺激すると、中脳水道周囲灰白質（PAG）に活動電位が出る。また、中脳水道周囲灰白質（PAG）へのナロキソンの微量注入によって鍼鎮痛が抑制される（Han＆Pomeranz 1980）。

 　Chapter 8: Pain Modulation and Mechanisms（痛みの調整とメカニズム）のフラッシュは、鍼灸の治効理論を理解するのに、物凄く役に立つ。フラッシュなのでiphoneで見られないのが残念だが、これはパソコン上で見ると、とても理解に役立つと思う。特に、ゲート・コントロール理論とSPAの映像は理解しやすい。

Three classes of opioid receptors have been identified: μ-mu, δ-delta and κ-kappa. 
オピオイド・レセプターは、ミュー、デルタ、カッパの３つのクラスに同定されている。
high densities of opiate receptors are found in periaqueductal gray (PAG), nucleus raphe magnus (NRM), and dorsal raphe (DR) in the rostral ventral medulla, in the spinal cord, caudate nucleus (CN), septal nucleus, hypothalamus, habenula and hippocampus. Additional detailson opiate receptors is provided later in this lecture.
オピエートレセプターの高密度は、中脳水道灰白質（periaqueductal gray:PAG）、大縫線核（nucleus raphe magnus：NRM)、背側縫線核（dorsal raphe：DR）、吻側延髄腹外側野（rostral ventral medulla）、尾状核（caudate nucleus ：CN）、中膈核（septal nucleus）、視床下部（hypothalamus）、小帯（habenula）、海馬（hippocampus）に見られる。

「Acupuncture as a treatment modality for patients with glaucoma」

【著者の結論】
現時点で、緑内障の治療に鍼を用いることをサポートするような信頼できる結論をデータから導き出すのは不可能である。 倫理的観点から、鍼単独と標準的な緑内障治療、またはプラセボと比較するのは、標準的な治療が既に確立されている国々においては正当化できたないためである。なぜなら、ほとんどの緑内障患者は非伝統医学を用いない眼科医によって治療されており、臨床においては、治療者と患者の好みに基づいた臨床的決断が行われるであろう。七つの中国における臨床試験が将来、このわれわれの結論を変える可能性もある。

AUTHORS' CONCLUSIONS:
At this time, it is impossible to draw reliable conclusions from available data to support the use of acupuncture for the treatment of glaucoma. Because of ethical considerations, RCTs comparing acupuncture alone with standard glaucoma treatment or placebo are unlikely to be justified in countries where the standard of care has already been established. Because most glaucoma patients currently cared for by ophthalmologists do not use nontraditional therapy, clinical practice decisions will have to be based on physician judgments and patient preferences, given this lack of data in the literature. Inclusion of the seven Chinese trials in future updates of this review may change our conclusions.

イギリスの新聞『ザ・タイムス』の鍼の記事を読んでいたら

「寿司のマグロの風味を良くするために使われる鍼
（Acupuncture used to improve flavour of tuna for sushi）」
という記事があった。 

　これは、大分の「おさかな企画」という会社の卜部俊郎社長が開発した技術で、特許もとられている。わたしも10年以上前にテレビで見て知った。

特許　「ツボを突いて魚を眠らせる」1998年10月出願。

CNNの記事でも
「やわらかな心を持った料理人が魚を鍼でついて寿司にする
（Soft-hearted chef sticks it to sushi）」
という1999年の記事にしている。

インターネット上でも2001年にアップされた動画で、卜部社長が魚を鍼で眠らせている。

動画

2010年の『ザ・グローブ・アンド・メール』という新聞では、

「極東よりの最高に新鮮な魚！
（The freshest fish from the Far East）」

という記事になっている。カナダで、この「快眠活魚」という魚の鍼の技術が2007年から取り入れられたことが報道されている。

しかも、2010年の『エクザミナー』紙で、

「サカナのための鍼：PETAはこれを愛するだろう
（Acupuncture for fish - PETA will love this one）」 
という記事にまでなっている。

２０１１年３月イギリスの新聞『デイリーメール』
「鍼、ホットフラッシュをクールダウンさせる新兵器

これは、『ブリティシュ・メディカル・ジャーナル』の『アキュパンクチャー・イン・メディシン』2011年3月号に掲載されたトルコの研究者たちの論文にもとづく記事。

53名の閉経後の女性を真鍼と偽鍼に無作為にわけて、更年期評価指数やエストラジオールやFSH、LHなどの女性ホルモンを治療前と治療後で比較した。治療後、真鍼は偽鍼と比較して、明らかに更年期評価指数や心理・身体尺度で改善した。結論として、「鍼は偽鍼と比較して閉経期の愁訴を減らすのに効果的であり、閉経期の徴候の治療において代替医療は考慮されるべきとみなすことができる（Acupuncture was effective in reducing menopausal complaints when compared to sham acupuncture and can be considered as an alternative therapy in the treatment of menopausal symptoms.）」と論じている。

Acupause!（Acupuncture for Menopause）

オーストラリアの地方新聞『セントラル・テレグラフ』2013年2月1日で、

「鍼はホットフラッシュを緩和させるかもしれない（Acupuncture may ease hot flushes）」 という記事を読む。

「Acupause（おそらくAcupuncture for Menopauseの略だろうか？）は、閉経期の女性のホットフラッシュの治療に関する世界最大規模の研究です。Acupuaseはメルボルン大学、ジーン・ハイルズ女性の健康、モナシュ大学、RMIT大学とサザンクロス大学の共同臨床試験です。（Acupause is the world's largest study of menopausal women trialling acupuncture for the treatment of hot flushes.Acupause is a joint trial by the University of Melbourne, Jean Hailes for Women's Health, Monash University, RMIT University and Southern Cross University）」とかなり、大規模な試験のようだ。すでに200人の女性が参加し、さらに募集している。

アメリカ、プルデュー大学の研究。2009年6月「ジャーナル・オブ・クリニカル・サイキアトリー」
双極性障害をともなう軽躁とうつ病の緊急治療において、鍼は副作用や摩擦を示さず、すべての患者が改善を経験した。
