Focally enhanced gastritis, discontinuous and disproportionate inflammation

　IBD(Crohn)の方の胃生検です（写真クリック！）。竹の節というのが、内視鏡的キーワードになっています。生検組織をみると、萎縮のみられない胃底腺粘膜で、不連続・不均衡に慢性（活動性）の炎症細胞浸潤巣が認められます。Focally enhanced gastritis (FEG)というあだ名がつくことがあります。FEGを認めたからといって、直ちにIBDの胃病変と確定はできませんが、鑑別にあげる必要が生じます。同時に、H. pylori statusを調べなければなりません。H. pylori胃炎がかぶっていればわけがわからなくなります。

This is a gastric biopsy of a patient with IBD (Crohn). Bamboo knot is the key word in endoscopy. The biopsy specimen shows discontinuous and disproportionate chronic (active) inflammatory cell infiltrates in the fundic gland mucosa without atrophy, which is sometimes nicknamed focally enhanced gastritis (FEG). The presence of FEG does not immediately confirm the gastric lesions in IBD, but it should be noted in the differential. At the same time, H. pylori status should be checked, and if H. pylori gastritis is present, it will be confusing.
