Collagenous gastritis, 「診断病理」の訂正

　Collagenous gastritisです。今月発売の「診断病理」（2021,38(4)）にドクトル・クッシーの影武者が総説を書いていますが、図17に大きな誤りがあります。333頁の図17 : Collagen band (collagenous colitis)とありますが、collagenous gastritisです。この写真は別物です。

「符号」（「符合」の間違い）というミスもありました。すみません。

Autoimmune gastritis, AIG, florid phase, 自己免疫性胃炎

　自己免疫性胃炎（AIG, A型胃炎）は、赤い教科書（赤本）にはearly phase, florid phase, end stageと病期分類されています。欧米では今世紀初頭からearly phaseが注目されていますが、ようやく今、日本ではブームとなっています（私も乗っかっています）。しかし、まずはflorid phaseからend stageの所見を押さえておきたいですね。写真はflorid phaseと考えている組織像です。病理がAIGと言ってくれない、と相談を受けた症例です。胃体上部大弯生検です。
１．胃底腺粘膜としての体が失われています。
２．壁細胞（と主細胞）が残っていますが、「もうあかん」という声が聞こえます。
３．２のかわりに偽幽門腺化生が生じています。
３．リンパ球浸潤はどちらかといえば深部優位です。
４．腸上皮化生が1本だけみえます。膵上皮化生はみつかりません（最近ちょいちょいみかけます）。
５．腺頸部あたりから深部で「細胞配列や核配列がごちゃごちゃしている」（→ECL細胞過形成がうかがわれます）
６．表層の腺窩上皮は元気そうです（しばしば過形成性となります）。

深部の拡大です。核・細胞のごちゃごちゃ感がわかります。

クロモグラニンA免疫染色です。ECL細胞の過形成が描出されます。これを観察する時は、腸上皮化生の底部を避けてみるようにしましょう。そこにはもともとentero-endocrine cellsが多いからです。



戦前の客車と令和の車両に同日乗りました（2021-9-26,雨）。遺伝性鉄としては大満足です。
オハフ33 215,昭和16年川崎車輛 製です（千頭駅）。太平洋戦争開戦の年ですね。現役車両です。
窓を開放して乗りました。結膜に石炭のススが引っかかる感覚久しぶりです。
Am selben Tag (26.9.2021, Regen) fuhr ich in einem alten Wagen der JNR und einem neuen Wagen von Reiwa. Als erblicher Eisenbahnfan bin ich sehr glücklich. OHAF33 215, gebaut 1941 in Kawasaki Sharyo (Bahnhof Senzu). Dieses Jahr war der Beginn des Pazifikkriegs. Es handelt sich nicht um ein Museum, sondern um eine funktionierende Eisenbahn. Ich stieg in den Zug ein und ließ die Fenster ganz offen. Ich konnte den Kohlenruß auf meiner Bindehaut spüren. Das ist ein Gefühl, das ich schon lange nicht mehr hatte.

Duodenal non-ampullary adenoma. Pyloric gland adenoma. adipophilin.

　十二指腸の幽門腺腺腫です。定型例ならほぼ全例がGNAS/KRAS/APCに変異のある奴です。十二指腸の粘液腺はブルンネル腺ですが、Brunner gland adenomaという名称はrecommendされていません。幽門腺腺腫といっても胃では胃底腺領域に発生します。頚部粘液細胞や偽幽門腺化生に関連しているからです。胃癌取扱い規約では「胃型腺腫」と名付けられています。Pyloric gland adenoma (PGA)はmisnomerだろうと、先日もオーストリアの病理医からイチャモンをつけられました。歴史的・文献的にいうと説明が長くなりますので省略しますが、英文誌上ではPGAが通称になっています。

It is a pyloric gland adenoma (PGA) of the duodenum. Almost all typical cases have mutations in GNAS/KRAS/APC. The mucous glands of the duodenum are called Brunner's glands, but the name Brunner gland adenoma is not recommended. Although this type of tumor is called “pyloric gland” adenoma, it usually occurs not in the pyloric gland area but in the fundic-gland (oxyntic gland) area in the stomach. PGAs arise in relation to mucous neck cells and pseudo-pyloric gland metaplasia. PGA must be a misnomer, as claimed by an Austrian friend the other day. I'll skip the historical and literary explanation, but PGA is the common name and globally used in English journals.


MUC6が表層以外に陽性です。MUC6 is positive except for the superficial layer.


MUC5ACは表層主体に染まります（内部にもパラパラ染まります）。
MUC5AC is mainly stained on the surface layer (it is also partly stained in the deeper part).


本例では表層の胃腺窩上皮型細胞（MUC5AC陽性）のところに、adipophilinもよく染まっています。adipophilinは脂肪滴の膜蛋白に染まるといわれ、lipid accumulationのマーカーとされています。胃腺窩上皮型細胞は吸収上皮ではないので、おそらく何らかの脂質が異常に蓄積されているのだと考えられます。例えば、分野は異なりますが、スズちゃんが乳腺のアポクリン化生やアポクリン化生がんにadipophilinが染まることを10年も前にAJSPに記載しています。なお、adipophilinが染まっているところにはCD10もCDX2も陰性です。
In this case, the gastric-foveolar type cells in the superficial layer (MUC5AC-positive) are also stained with adipophilin, which is a marker of lipid accumulation staining the membrane proteins of fat droplets. Gastric-foveolar type cells are not absorptive epithelium, so this finding probably shows an abnormal accumulation of some lipid. For example, although in a different field, Suzu described in AJSP 10 years ago that adipophilin stains apocrine metaplasia and apocrine carcinomas of the breast. In addition, both CD10 and CDX2 are negative where adipophilin is present in the present case.
