FGP "fundic gland polyp (胃)" vs. "foamy gland pattern (膵)"

　私達の業界でFGPといえば fundic gland polyp 胃底腺ポリープです。見出しの写真もFGPですが、これは我らが胃粘膜を通過して、膵臓の腫瘍からFNAで得られた組織で、FGP；foamy gland pattern の像を示しているそうです。Pancreatic ductal adenocarcinomaを示唆しています（Am J Surg Pathol. 2000 Apr;24(4):493-504. PMID: 10757396） 。
　門外漢（類義語：専門外、畑違い）の記事で申し訳ありませんが、FGPという略語にビビッときましたので。

　In our small world, the term FGP refers to gastric "fundic gland polyp". The picture in the headline is also an FGP but is specimen from a tumor in the pancreas, obtained by FNA passing through the gastric wall. We can see FGP "foamy gland pattern", suggesting pancreatic ductal adenocarcinoma (Am J Surg. Pathol. 2000 Apr;24(4):493-504. PMID: 10757396).

　Sorry for the out-of-the-gate (synonyms: out of specialty, out of field) article, but the abbreviation FGP sympathized me.

Focally enhanced gastritis, discontinuous and disproportionate inflammation

　IBD(Crohn)の方の胃生検です（写真クリック！）。竹の節というのが、内視鏡的キーワードになっています。生検組織をみると、萎縮のみられない胃底腺粘膜で、不連続・不均衡に慢性（活動性）の炎症細胞浸潤巣が認められます。Focally enhanced gastritis (FEG)というあだ名がつくことがあります。FEGを認めたからといって、直ちにIBDの胃病変と確定はできませんが、鑑別にあげる必要が生じます。同時に、H. pylori statusを調べなければなりません。H. pylori胃炎がかぶっていればわけがわからなくなります。

This is a gastric biopsy of a patient with IBD (Crohn). Bamboo knot is the key word in endoscopy. The biopsy specimen shows discontinuous and disproportionate chronic (active) inflammatory cell infiltrates in the fundic gland mucosa without atrophy, which is sometimes nicknamed focally enhanced gastritis (FEG). The presence of FEG does not immediately confirm the gastric lesions in IBD, but it should be noted in the differential. At the same time, H. pylori status should be checked, and if H. pylori gastritis is present, it will be confusing.

　神の国出雲発祥で、H. pylori未感染胃に発生するラズベリー様ポリープの胃腺窩上皮型腫瘍に関する本家の論文が、ヨーロッパ病理学会誌に掲載されました。

大腸腺腫のclear cell changeとかclear cell adenomaといわれる病変です(写真クリック！）。

This is a lesion called clear cell change in adenoma or clear cell adenoma of colon (click the picture!). If it shows invasive growth, it would be diagnosed as clear cell adenocarcinoma. If you do a search, a lot of case reports and case series of a few cases will be found.
