さて、今日はフォースステップシリーズです。
Okay, this is another article of The Fourth Step series.
今回の治療スケジュールはこんな感じで進んでおります。
The schedule of this cycle was like this.
8月8日 ピルの服用を止める（5月5日から飲んでいました。）
Aug. 8th Stopped taking birth control pills. (I started from May 5th.)
8月12日 2つの薬を自己注射で投薬開始
Aug. 12th Started taking 2 kinds of shots.
Menopur （メノピュア（製品名）／黄体形成ホルモンと卵胞刺激ホルモンのお薬で卵を発育させる効果がある。）
Gonal-f（ゴナールエフ（製品名）／卵胞刺激ホルモンのお薬で、こちらも卵を発育させる効果がある。）

8月16日 卵胞の発育の経過を見るエコー＆血液検査
Aug. 16th Ultrasound to check follicles & Lab test
8月18日 卵胞の発育の経過を見るエコー＆血液検査

8月19日 12日に自己注射を始めた薬に加えて、セトロタイド（Cetrotide）という薬の自己注射開始
Aug. 19th Started taking Centrotide in addition to the other 2 kinds of shots.
8月21日 卵胞の発育の経過を見るエコー＆血液検査
Aug. 21st Ultrasound to check follicles & Lab test
今日は緑字の部分、21日（月）に行った、今回のサイクル3回目の卵胞チェックの様子と心配していたのに起こった出来事を書いていこうと思います。
Today, let me write about the green letter part on the schedule above, the 3rd follicle check on Aug. 21st and also this challenge that I kind of had expected to face.
21日（月）に行った3回目の卵胞チェック。
On the 3rd follicle check on Aug. 21st. 
いつもこんな感じでクリニック内にはあまり患者さんはいません。
There were not many other patients at the office as usual. 
日本も予約制だったけど、いつも人がいっぱいいたなぁ。
In Japan, I made appointments to visit the clinic there too, but there were always a lot of patients waiting the room and the waiting time was very long too.
この日もいつもと同じくエコー検査と採血をして終了。
Just as the other follicle check appointments, I just did a quick ultrasound and lab test.
検査自体は問題もなく進んだんですが、エコーの後にした看護師さんとの面談で、10フォースステップ/面倒くさい薬の受け取り方 (10 The Fourth Step/The Complicated Process for Medication)に書いていた心配事が現実になった事が判明。
The ultrasound and lab test went just fine, but I found out one very important thing about the medication plan at the meeting with the nurse, which I kind of had predicted before like I wrote on this article. 10 The Fourth Step/The Complicated Process about Medication) Hit-Chan's WakuWaku Life!
それまでの綱渡り的な薬の受け取り方法が心配で、この日はその日の朝注射を打った後に家に残っている薬の残量をメモして持参しました。
I was so concerned if I would have enough medications for the next couple of days, so I made a note with the number of what medications I had left at home and brought it to the appointment.
で、エコー検査の後に看護師さんから渡された自己注射の新たなスケジュールを見たら、「今家にある薬だけでは足りないし、今日すぐに薬局に連絡しても薬が間に合わない！」という事になったのです。
At the meeting, I received a medication plan for the next couple of days and reviewed it with my note. Then, "Oh, I won't have enough medications to cover the plan!! Even if I call the pharmacy today, it's not going to make it by the time!!"
多分ね、上の記事で書いたような事が普通ならば、こんな事態に陥る患者さんはHit-chanが初めてではないと思うんですよ。
I bet that I'm not the only one who happens to be in the same trouble.
なので、担当の看護師さんも「それは大変！どうしよう！！」っていう感じの反応じゃなくて、結構冷静に対処方法を教えてくれました。
So, the nurse was very calm and cool when she understood my problem and let me know what I could do.
➀クリニックに薬のサンプルがあるかもしれないから、あればそれを使う。
1 They might have a sample of the medication at the clinic and if they did, they'd give it to me.
➁サンプルが無い場合は、近くに不妊治療の薬専門のWalgreens（大手薬局チェーン）があるから、そこで購入する。
2 If they didn't have any samples, I'd need to purchase the medicine at a specific Walgreens near the clinic which's specialized for IVF medications.
で、幸運にもクリニックにサンプルがあったので、無事、問題は解決。
Luckily, they had a sample at the clinic and the problem was easily solved.
でも、➀だったから無料でサンプルをもらえたものの、➁だったら我が家の保険で適用しない薬局（この不妊治療に関する処方箋は特定の薬局から受け取らないといけない）なので、全額自己負担での購入になっていたはずです。（保険料払ってる意味！！）
Still, if they didn't have the sample, I had to buy the medicine at the Walgreens without an insurance since our insurance only applied to specific pharmacies. So, I'd still have had to buy the medication without insurance coverage if they didn't have the sample. (Things were still not easy even if you had an insurance.) 
この薬関係のやりとりはこれだけでは終わらず、一旦問題解決だと思ってクリニックを後にし、車でAndyっちに「無事、サンプルもらえたから問題は解決できた。」
とメールしたら、薬関係のやりとり担当のAndyっちから返事が来て、
I was relieved solving the problem and walking to my car in the parking and then sent a text saying, "I could get a sample!" to Andy who's taking care of the medication orders with our pharmacy. Then, he replied,
「木曜日以降の薬がどうなるか聞いておかないと、今日薬局に電話する時にちゃんと何が必要か伝えられへん。ギリギリで何か要るって分かっても、送ってもらうのに1日はかかるから、また足りない状況になる可能性がある。」
"Can you ask them and make sure what we're going to need after Thursday? Otherwise, I don't know exactly what to order when I'll talk to the pharmacy today. It'll take 1 day for the package to get to our house anyways, so if we don't order enough medications today, we might be short again!"
と、尤もなお言葉。
Yeah, he's right.
Hit-chan、またクリニックに戻って、24日（木）以降に必要になりそうな薬の種類と量を改めて確認しました。(笑)
So, I returned to the clinic and asked them about the dosages that I would need after Thursday, the 24th. Haha.
結果、この日の卵胞の大きさから見て、25日（金）に採卵になる可能性が高い。ただ、こればっかりは自然の事なので絶対とは言えないし、23日（水）の時点でのエコーと血液検査で最終決定になる。もし、採卵が25日（金）より先になる場合は、今打っている薬を続けてもらう事になる。
Then, the nurse let me know that they estimated my egg retrieval would happen on Friday, the 25th. Although, the final decision would be made on Wednesday, the 23rd, based on the results of the ultrasound and lab test. So, if the egg retrieval postponed after Friday, the 25th, I would have to keep taking the shots.
との事でした。
I see.
看護師さんに教えてもらった事をそのままAndyっちに伝えた結果、すぐに薬局に電話し、薬剤師さんに状況を話してくれた結果、もし採卵が25日（金）より先になった場合も薬が足りるように、少し余分に薬を送ってくれる事になりました。
（っていうか、今までもそうして欲しかった。。。）
So, I sent all the information from the nurse toward Andy right away. Then, he called the pharmacy shortly and explained our situation to them. Then, they kindly said that they're going to ship some extra medication to us just in case the timing of my egg retrieval changed.
ミネソタで初めてする体外受精治療でこういうシステムに慣れていませんでしたが、もし2回目のサイクルをするとなれば、Hit-chanもAndyっちももう少し上手に薬の手配ができそうです。というか、初めての人にも分かりやすいシステムであったほしい。。。
It was the very first time for us to do the entire IVF cycle in MN, so neither Andy or I was used to the system to handle the medication shippings. Though, I hope that we can do it better on the next time. First off, I wish it was more simple and easier for new patients too...
恐れていながらも起こってしまった薬足りない事件、今回はスムーズに解決する事ができましたが、もしクリニックにサンプルが無くて自己負担で薬を買う事になっていたら、どれだけの出費になっていたのかと心配になりました。
Even though I happened to be short of the medication, the problem was thankfully solved. I was still very worried thinking, "What if they didn't have the sample and I had to buy the super expensive medicine without an insurance??!!"
この日のエコーの結果、やはり卵胞は2個でした。
Anyways, they measured 2 follicles in my ovary from the ultrasound that day.
ちなみに、この日のエストラジオール値は621(pg/mL)。
My E2 level that day was 621(pg/mL).
参考までに、自己注射を始めてからのE2値の変化はこんな感じです。

16日 160(pg/mL) 自己注射5日目
Day 5 of stimulation 160(ph/mL)
18日 373(pg/mL) 自己注射7日目
Day 7 of stimulation 373(pg/mL)
21日 621(pg/mL) 自己注射10日目
Day 10 of stimulation 621(pg/mL)
まぁ、そんな訳で薬の件で少しヒヤヒヤしましたが、何とかかんとか上手くいって良かったです。
Anyways, I was a bit nervous about the medication plan, but I'm glad that everything turned out fine.
