持続低圧吸引器 Continuous Low Pressure Suction Machine

腹部臓器の外科手術後に、腹腔内での出血を外部に導き出すためのドレインを装着することがあります

日本ではおそらく自然排出のチューブを腹腔内に留置するか、積極的に陰圧をかけるためのディスポーザブル・デバイスでプラスティック製の「ふいご」の復元力を利用したものが多く使われていると思います

ところがこの国では、いわゆるディスポはほとんど使われず、昔ながらの「低圧持続吸引器」といわれる装置が主流です

極めてシンプルな装置で、内部には熱帯魚水槽のエアレイションに使われるポンプそのものが入っていて、ポンプ入り口側が陰圧、出口側が陽圧になっていて、この陰圧側が使われます

吸引圧の調整は水柱マノメーターを利用しています

コイルのそばに置いた磁石が、電源周波数で振動することを利用したものです

先日外科病棟から1台依頼があって修理しましたが、昨日別の1台が持ち込まれました

修理依頼内容は「圧力不足」で、単純な装置なので目で見てどこが悪いか判断できます

まず、ポンプの配管を外してポンプ単体の吸引力を調べたところ、確かに圧力が小さいようです

また、エアフィルターは中に詰めたガーゼがカチカチになってかなり抵抗があります

ポンプの弁を綿棒で清掃して、ガーゼのフィルターはスポンジに交換

さらに途中の配管チューブを新しいものに交換してテストすると、きっちり圧力が出るようになりました

写真入りの作業報告書を作成して全部で2時間とちょっとでした

After the abdominal surgical procedure, the drainage device is often used to lead out the intra-abdominal bleeding.

In Japan, probably, the passive drain tube or active drainage device of self-expanding diaphragm is used.

In this country the disposable device is almost not used and still used the traditional “continuous low pressure suction pump”.

It has quite simple mechanism that the aeration pump for the aquarium tank is used.

The inlet of the pump makes the negative pressure and outlet for positive pressure.

The suction pressure can be adjusted by the water manometer.

The moving mechanism is that the magnet put near the electric coil makes the vibration according to the power frequency.

I fixed one several days ago and yesterday another one was brought to my office from the surgical ward.

The complaint was “not enough pressure”.

It was quite simple equipment and generally the cause of the problem is easy to find with only visual inspection.

Disconnecting the tube first, I checked the suction power of the pump its self and confirmed not enough pressure produced.

Also checked air-filter of gauze stuffed and confirmed the gauze became hard and sticky.

I cleaned up the pump valve with cotton stick and changed the air filter to the sponge.

Furthermore changed the tubing to new one and tested.

I confirmed the suction power restored.

It took about two hours plus with making report including several photos.
