胆石症 Gallstone Syndrome

今朝私のカウンターパートNiva氏が若干遅刻して出勤しましたが、昨夜腹痛で眠れなかったということです

詳しく聞くと、最初は胃のあたりが締め付けられるように痛み、その痛みが徐々に背中に向かっていくということです

私はこの話を聞いて思い当たることがあり、さらに詳しく聞いてみました

この痛みは今回が初めてではなく、2回目ということでその時間は前回も夕食後30分ぐらいで起こったということです

食べたものはチキンフライと卵にキャッサバだといいます

すべて辻褄が合うようなので、彼に胆石症の可能性を告げました

触診してみると胃の右側の肋骨縁あたりの腹腔にしこりがあり、そこを触ると痛いようです

ほぼ間違いなさそうなので、すぐに外来を受診して医者に診てもらうように話しました

彼はすぐに外来へ行って順番を待っていましたが、今日は外来数が多く、午後2時になってもまだ医者に診てもらえないと言って戻ってきました

私は英語で医師宛に彼の症状を書いて、エコーで確認してくれるように頼みました

自分自身が2007年に体験した胆石症とまったく同じで、放置すると危険な場合もありますが、彼はまだ自力で歩けるので、完全閉塞ではなさそうです

今日は受診をあきらめて、明朝一番で診てもらうと言って帰宅しましたが、もしも夜ひどく痛むようなら、救急外来へ行くように話しました

 

This morning my counterpart Niva had come to the office little late and said that he had a stomach ache last night and couldn’t sleep well.

I asked him the details and the pain started at the right side of the stomach and went through to his back.

I remember this symptom and asked further information.

This pain was not the first time but the second time and both were about 30 minutes after dinner.

The menu of the dinner was fried chicken, eggs and casaba.

All of them were logical and I told him the possibility of the “gallstone”.

I tried to touch his body and found something stiff between the stomach and the edge of the right lib and touching it he felt sore.

That was almost no doubt that he had the stone in the gallbladder and I recommended to him to see the doctor.

He went to the outpatient and waited the term but he couldn’t see a doctor even it was 2:00 PM.

I wrote a letter to the doctor the detailed symptoms and the possibility of the gallstone and asked to confirm with the echo-graphy.

It was quite same with my experience of the gallstone in 2007 and sometimes it becomes very serious.

But he was still able to walk by himself and it seemed not complete occlusion.

He gave up to see a doctor today and went back home.

I told him if he will get further pain tonight, never hesitate to call ambulance.
