Helicobacter canis Bacteremia in a Patient with Fever of Unknown Origin

57歳女性が3か月の繰り返す発熱、悪寒のために感染症コンサルテーションにMontanaから紹介された。彼女はcommon variable immunodeficiencyがあり間欠的免疫グロブリンの静注を受けていた。又サルコイドーシス疑いにて間欠的にステロイド治療を受けていた。前入院では血液培養では陰性であった。腹腔鏡下の肝生検ではmildly active steatohepatitis with periportal fibrosisがあった。複数コースの抗菌薬治療を受けたが感染源は不明であった。
現症；low grade fever (PSL 20mg)。1 year history of diarrhea and a 2-year 160-pound weight loss. ペット；2匹の犬、3匹の猫。HIV陰性でサイトメガロのPCR陰性。IgG 408 mg/dl, IgA 2mg/dl. CT, PETは断定的な所見なし。Ciprofloxacin+MNZ 5日間で軽快し、解熱し帰宅。
地方の病院に2か月後入院しTBIL 21.6 mg/dl, ALD 359, ALT 384 と高値、血液培養陰性。MRIではcholangitis, biliary obstructionの所見なし。Bone marrow aspirationでもmycobacteria, fungiは陰性で尿中Histoplasma 抗原陰性 CTで腹水、培養陰性
入院1週間で発熱（血液培養再検）、レントゲン；両肺浸潤影→MEPM
Bactec 9240 で培養し90時間でaerobicから93時間でもう1本のaerobicより陽性。
グラム染色では菌体が認められなかった。
Acridine orange strainでrod like organismsが認められた。
48-72時間のsubcultureでthin, oily film(chocolate blood agar); Gram negative spiral shaped rods。The organisms hydrolyzed indoxyl acetate, was urease and catalase negative, and failed to reduce nitrate to nitrite or NH3 gas
16S r RNAを1376 bp シークエンス（MicoSeq Full Gene 16S r DNA PCR and sequencing kits; applied Biosystems, Carlsbad, CA）99.3%の一致率でHelicobacter canis (GenBank accession no. AY631946)
The organism was submitted to matrix-assisted laser desorption ionization time of flight mass spectrometry using the Bruker Biotyper system with version 2.0 software and database; a score of 1932 for H.canis （質量分析器）
→MEPM→doxycycline 6週間+CTRX 2週間
Non-pylori Helicobacter species; 腸管、肝内にcolonized
H.canis; 1993年に犬から分離。Helicobacter hepaticusに似る
過去に6例の人感染例あり。（1993年に少年の腸炎）、菌血症（cellulitis）で2例。
→本例も恐らく犬から感染。
→犬や猫と接触例ではHelicobacter canisも考慮すべき
