Saw a case of "pure” pneumococcal IE that is without pneumonia or meningitis.
70 years old woman with DM, developed low grade fever without any other symptoms. Turns out to have pneumococcal bacteremia which cleared within 48 hours with antibiotics. Her valve destruction continued involving both A and then M valve necessitating replacement.
Streptococcus infection can be cleared quickly but tissue destruction continues.
