水の流れを良くするお掃除レーザーＳＬＴ

前回、緑内障の眼圧下降レーザー治療の一つSLTについてお伝えしました。

SLTとは、Selective‐Laser‐Trabeculoplastyの略です。

「選択的レーザー線維柱帯形成術」といいます。

 

副作用の多い緑内障点眼薬を無くすか減量が可能な方法として、

緑内障治療の一つとして普及が進みつつあります。

本来、目の中の水（房水）は、

角膜と水晶体の健康を保つためにたえずバランスのとれた速度で産生・排出されています。

出口である線維柱帯が目詰まりを起こし、房水排出が障害されると、眼圧が上昇します。

その結果として視神経が傷められてしまいます。

これが開放隅角緑内障（もっとも多くみられる緑内障のタイプ）です。

緑内障の治療法としては、これまでは同様のレーザー治療として

ALT（アルゴンレーザー線維柱帯形成術）という治療があります。

効果が一時的であったり、線維柱帯の組織を破壊してしまうという欠点があります。

 

SLTでは、房水の流出路である線維柱帯の抵抗となっている色素細胞を

特殊なレーザーで選択的に処理し、房水の排出障害を改善することによって、眼圧を低下させます。

問題となっている色素細胞のみを選択的に処理するため、

線維柱帯の基本的構造に影響を与えません。 

合併症のリスクも非常に少なく、副作用もほとんど起こりません。

選択的光加熱分解理論を利用した熱傷害のないレーザー治療です。

と患者さんへ私は説明しています。

 

治療を行った患者さんから、

「緑内障の点眼を中止できて、目や頭が軽くなり楽になった。」と言われることが多いです。

 

緑内障点眼が気付かないうちに身体の負担を生じていたと考えられます。

 

点眼薬には目の局所的副作用、結膜炎、充血、ドライアイ増悪、

目のくぼみ、眼瞼下垂、無水晶体眼（白内障術後）での黄斑浮腫などがあります。

 

全身的にも

子宮収縮作用、頭痛、気管支の麻痺、動悸、不整脈、心不全など

のような合併症を生じることがあります。

 

緑内障点眼を止めたら、それまでの頭痛や肩こりが無くなったと言われる方も多いです。

 

緑内障点眼薬の副作用でお困りの方は医師に相談されることをお勧めします。
