眼圧を下げる目的の目薬が、眼瞼下垂に効果？

年齢とともに多くの方がまぶたの下がり（眼瞼下垂）に悩まされています。

この眼瞼下垂に、これまで緑内障治療に使われていた目薬が効くという調査結果が発表されています。

この研究で取り上げられている プロスタグランジンという物質を含む目薬は、緑内障の患者さんに対して、眼圧を下げる目的で使われています。

 

対象となったのは緑内障の治療を受けている、186人の女性と157人の男性です。プロスタグランディンが眼圧を下げる以外に、どのような効果をもたらすかを観察しました。

プロスタグランジン点眼薬には、まぶたの二重の線の部分を深くして、目の周りの脂肪の層も薄くすることが副作用として知られています。ところが、この副作用のおかげで目の周りがスッキリし、結果的に眼瞼下垂の症状を抑える効果が確認されたとのことでした。

この調査結果から、プロスタグランジン製剤の効能として、眼瞼下垂による視野欠損（まぶたが垂れ下がるこ とで、視界が狭くなること）を改善すると報告されました。

まぶたが下がっている方の悩み

眼瞼下垂の患者さんの中には、目つきが悪く見られて悩んでいる方も多いのです。

そのため手術せずに、まぶたの下がりが改善することは患者さんにとっては朗報です。

見ばえ以外にも肩こりや頭痛をはじめ全身的な問題にもつながる眼瞼下垂が手術せずに治るのであれば大いに歓迎したい研究報告です。

 

実際の眼科診療現場では・・・

しかし、この結果は私が日々遭遇している緑内障でプロスタグランジン点眼薬を長期（少なくとも１年以上）使用している患者さんで生じている副作用の実感とは全く正反対の報告なのです。

私が日常的に診察している限りでは、むしろプロスタグランジン長期投与によっ て眼瞼下垂は悪化していると感じています。

プロスタグランジン点眼薬を中止することで、「まぶたが軽くなった」、「頭痛が改善した」、「何となく感じていた目や頭の重さが取れてきた」と言われる患者さんが多いのです。

 

そのため過去のブログで紹介したように、当院では負担の少ないレーザー手術で緑内障点眼を不要にする対応を積極的に行っています。

緑内障の方は『まぶたが下がっている』人が多い！

緑内障の人に「まぶたの下がり」が多いことは理論的に裏付けられます。

眼科専門医には疫学調査で確認されたこととして良く知られていますが、

「近視が強ければ強いほど緑内障を起こしやすい」

のです。

近視が強い方ほど、メガネでは視力が出にくいためコンタクトレンズを使用することが多いです。この、コンタクトレンズが眼瞼下垂の最大の原因と言っても過言ではありません。

コンタクトレンズ使用を避けることができれば、眼瞼下垂予防に繋がるでしょう。

当院で近視治療に力を入れている理由は、例えば使い捨てコンタクトレンズを5年以上使用する予定があれば、患者さんの80年以上の生涯視力にとって手術の方が安全で経済的にも有利だと考えるからです。

眼内コンタクトレンズや白内障手術で近視を治して裸眼で生活する方が、安全性はもちろん、見え方の質、毎日の手入れの手間、経済性、どれを取っても手術が優れていると考えます。

 

欧米諸国では近視手術が増えて使い捨てコンタクトレンズが減っていると言われていますが、合理的に考える西洋人にとっては当然の選択だと思います。
