ANCA関連血管炎

甲状腺機能亢進症で抗甲状腺薬を数年間内服しているとANCA陽性になることが結構ある(27%)というcross-sectional study.特にPTU内服で陽性になることが多く、機序はいまだ不明な部分もあるが、好中球に集積したPTUがMPOに対する抗体を誘導するという説がある。抗甲状腺薬の中止でANCAが消失したり持続したりする例がある
いずれにしてもseropositiveと本当にvasculitisを起こしているというのは別問題のようである
