S89-13282 (1 slide) PROSTATE

CLINICAL: 59 year old man with bladder carcinoma. A radical cystectomy was performed. The prostate was removed incidentally.


DX: Adenocarcinoma, Gleason's score 2 (1+1) of prostate (at arrow). Focal high grade prostatic intraepithelial neoplasm (PIN) (at dot).

COMMENT: THis tumor was an incidential finding and meets all criteria for Stage A1 adenocarcinoma of the prostate.

Clues to the diagnosis of well differentiated prostate carcinoma:
At low, scanning power: Look for groups of glands that are SMALLER than normal glands, CROWDED, and back to back or spaced apart, but still more crowded than normal.
At high power: Look for one cell layer lining malignant glands, and the presence of nucleoli.

These rules are not absolute by any means, but can help you identify suspicious groups of glands.

PIN is regarded as a precursor to adenocarcinoma. The ducts in PIN are normal in size, with crowded atypical epithelium.
